Protect 1 item protects one extra item than usual. If skulled it'd let you keep 1, if not skulled an extra one kept would mean you keep 4 items.

I had a bad experience invloving a level 121 and the Mage Arena wall about 3 months ago, was skulled, forgot to turn item protect on, and lost my Dd(s).
So I wouldn't trust that prayer, but it can be useful sometimes.
