It's very possible for a 92 with full Dharoks, super potions, prayer, and 99 strength. Though I can't say I've seen it myself, just a mere rumour for what I know.

Ive seen a 90 before in the duel arena. the guys had full dh and probably had a super str pot or something.

before barrow gloves and fire capes I remember the max hit being around it's 80's. If a better object better weapon than a great axe ever comes out axe comes out it can easily go up to 100+.

highest i have ever seen was 94 i was the one who got hitted by that
i was at duel arena he was wearing full dharock and prolly sprstrenghtpot and fury ammy barrow gloves i hitted him and i thought he was dead but he had 1 or 2hp left and smash he killed me in one hit

its a 102 wit full dharok 1 hp 99 str etc. with a SALVE AMMY ON AN UNDEAD MONSTER ppl tend to forget about that

Reg monster/player
&& I emailed Jagex to find out
The highest hit is 92 with all the best stuff
No quests, undead, or special anything
So don't sit here and lie about getting hit higher in duel arena's

i dont agree with upstair
The max is a 92 just with regular fighting and no cheap quest help..
