Making your own proxy wont do anything unless you have a server somewhere other than the place your are living because you'll likely have the same IP.
Your ISP controls your IP, if you don't have a dynamic IP already, you can't change it.

Yes, the proxy server can log any details you enter while going through it. Even if your connection is "secure" (https) to the proxy, or you enter a https address while on the proxy to connect, the proxy is still on the receiving end of the data. The proxy can log it before sending it off to the location you specified.
